Sixty percent of our health is affected by our social needs. Our access to food, mental health services, and community resources all play a pivotal role in improving our health outcomes. Yet, the groups that are financially at-risk for patients don't have the tools or data needed to manage these needs or hit expanding quality measures under the ACA. This inability results in an additional $85b a year in Medicaid spending. Our platform allows Medicaid Managed Care Plans (insurers) and providers to effectively address the social needs in their population to improve quality while reducing the cost of care. We deploy a web-based tool to identify the social needs in a population and subsequently connect patients to the best resources to address their needs. We then automate follow up with patients via interactive texts and present this unique set of patient data back to staff via a population dashboard. Scalable case management for better managed care!
